Care Coordination

Clients work with a care coordinator who advocates for them and helps them overcome obstacles they may face in obtaining community services. Our Care Coordinators can help with: the navigation of medical care, food assistance, emergency housing support, and medication assistance. After you apply below, we’ll determine what care program fits your needs (FAITH or RJ)

What our Care Coordination looks like

Fast Affirming Innovative Testing and Healthcare (FAITH)

We offer services ranging from STD/STI testing to Trans Gender Non-Conforming (TGNC) care to our clients through our network of gender-affirming physicians.

Reproductive Justice (RJ) Program

The RJ program provides reproductive care for birthing people. We provide emergency contraception and family support, such as infant care items, household supplies, and wellness items to those in need.

Food Pantry

The food pantry is open by appointment only to active FAITH and RJ members.
